小多andCursor ca4b09806a Initial V1: C2C Bridge with read-only MCP, OAuth+pairing, tunnel, CLI and Skill

ChatGPT thinks, Codex works: a loopback bridge exposes the current workspace
to ChatGPT through 8 read-only MCP tools over Streamable HTTP, protected by
OAuth 2.1 (PKCE, DCR, refresh rotation) with a one-time pairing code for
zero-input authorization, published via a Cloudflare Quick Tunnel.
Includes the c2c CLI (setup/start/doctor/pair/unpair/record), the Codex
Skill, the C2C agent protocol docs, and 76 unit/integration tests.
